It's the same as in a 757. It's not as bad as it might seem...I've done it numerous times on both the 737 and 757, however I will admit that, generally speaking, I prefer a widebody. From my experience, Northwest's 757-300s are a pretty comfortable ride, as are Aloha's 737-700s. The one widebody I do not recommend is Air Canada's 767-200 if you're in the forward coach cabin. The seat pitch is very cramped. However, the rows behind the overwing emergency exits aren't nearly as bad.
BTW, Alaska Airlines actually runs longer routes with their 737NGs than all these West Coast-Hawaii runs. Try Chicago-Anchorage or Miami-Seattle sometime ;-) Air Pacific also runs a 737-800 from Honolulu-Nadi....3,162 miles. I believe Copa runs a 737-700 on some Latin/South American route that's in the neighborhood of 3,200 miles. Plus, you can't forget about those Privatair 737BBJs across the Atlantic, but those don't exactly have your typical airline cabin interior ;-)
