I have the same display case and I think you may have over tightened the bolts that go on the threaded ends of the shelf supports. You know, the metal rack that goes from top to bottom and the wood on the top and bottom of the finished case screw to it to hold it all together. If there is the tiniest flaw (pressure point) in the glass and excess pressure is applied, well you know what happens. If it were me, I'd go to a glass shop (window glass) and have them cut me a new panel from salvaged glass. Most shops save any half decent pieces of the plate glass when they are replacing a store glass door or window. They use it for just such purposes, all they have to do is check to make sure the piece doesn't have any bad scratches on it and they'll cut it and sand the sharp edges for you for 50% or less than what you would pay for brand new glass. Just make sure the thickness is the same so the glass panel will sit down in the channel in the wood top and bottom.

I've done this in the past, as a matter of fact, I had 4 more metal cross bars that the shelves sit on, welded to my case so I now have 7 glass shelves. I display 1/72 tanks so I didn't need 15" between shelves, 7.5" works great. When I had the welding done I spray painted the new braces with aluminum paint and had 4 glass shelves made from salvage. It looks just fine. Good luck.
